I acknowledge the potential risks associated with the professional hair services I have requested at Hair By Lorena, located at 7B Maple Ridge Cres., Ottawa, Ontario, Canada. These services include but are not limited to balayage, highlights, color correction, perms (female and male), keratin treatments, and hair extension installation (all methods).
I understand that these services may result in unintended consequences such as:
Hair damage, dryness, or breakage
Allergic reactions or sensitivity to products
Scalp irritation, redness, or soreness
Tangling, shedding, or slippage of hair extensions
Uneven color results or the need for additional sessions
I further understand that certain factors may increase risks, including medications, previous chemical treatments, box dyes, over-the-counter products, and existing medical conditions. I have had the opportunity to discuss potential risks with my stylist and ask questions prior to receiving my service.
Despite understanding these risks, I voluntarily choose to proceed with the requested services.
I agree not to hold Hair By Lorena, my stylist, or any affiliated persons responsible for any unintended or unfavorable results, allergic reactions, or other conditions that may occur. I also release the salon from liability for damage to clothing, jewelry, or personal belongings during my appointment.
By signing this waiver, I release and discharge Hair By Lorena from any claims, demands, or legal actions arising from the services provided. I confirm that I have had sufficient time to review this document and understand that I am accepting it freely and voluntarily.
