I understand that these services may result in unintended consequences, such as increased risk of hair damage, allergic reactions, scalp irritation, burning, redness, soreness, and other side effects. Hair By Lorena is not liable for any damage to clothing, lost jewelry, or property not belonging to the salon during color services.
I am aware that certain medications, over-the-counter hair dye, and various medical conditions may heighten the risk of unfavorable outcomes. Having been informed of the potential risks, I have had the opportunity to inquire about adverse effects and risks from my stylist and conduct further research. Despite understanding the risks, I choose to proceed with the requested color correction services.
I accept the risks and agree not to hold my stylist, the salon, or any other person responsible for unintended results, allergic reactions, skin irritation, or any other condition that may arise.
By signing this document, I release and discharge Hair By Lorena, from any actions, suits, claims, demands, or damages resulting from unintended or unfavorable results or skin irritation arising from color correction services.
I confirm that I have had sufficient time to consider this document and seek independent legal advice if desired. I acknowledge that I am accepting this acknowledgment and release freely, voluntarily, without duress, and understand its limitations on legal actions against the releases.
